Wearing a shirt with a simple biological fact is not protected. Also, spray painting someone else's property with disgusting "art" is protected.

It works also in the other direction. A federal judge in Seattle struck down a law that prohibited graffiti, arguing it was ‘over broad,’ that criticizing police by spray painting buildings (vandalism) was protected speech.
